What happens beneath the skin

Subcision uses a small needle or cannula under local anaesthetic to separate fibrous bands that pull a depressed scar downward. The release may allow the surface to look more even as healing occurs. Saline is a sterile fluid; it does not replace the mechanical release and is not a filler. Bruising, swelling, tenderness, and temporary unevenness can follow the procedure.

The method is most relevant to selected rolling or tethered scars. Deep boxcar scars, ice-pick scars, active acne, infection, and scars with a different cause may need another approach or a staged plan.

Preparing and recovering

Tell the clinician about medicines that affect bleeding, allergies, active skin disease, previous abnormal scars, and any recent procedures. Follow instructions about cleansing, makeup, exercise, and blood-thinning medicines. Keep small entry points clean, avoid pressing or massaging the area unless instructed, and use sun protection while bruising or redness remains.
