What should you know about robotic surgery for spinal stenosis?

Spinal stenosis is a narrowing around the spinal cord or nerve roots. In the lower back, this can contribute to lower-back pain, radiating leg pain, leg numbness and tingling, or difficulty walking. The operation may aim to remove tissue or bone that is pressing on nerves. If a vertebra is unstable or has slipped, the surgeon may also discuss stabilisation with screws and a fusion.

A robotic system usually uses planning software, the operation’s imaging, and a guided arm or navigation tools to help place instruments or implants along the planned path. It does not independently diagnose the cause, choose the operation, or make decisions during the procedure. Some spinal stenosis operations do not need robotic assistance, and a conventional approach may be equally appropriate.

The possible advantages depend on the procedure and the team’s experience. Guidance may support accurate planning and implant placement, while the actual benefit for you depends on whether surgery addresses the source of nerve compression. Ask which part of the operation will be robot-assisted and what outcome the team expects it to improve.

Bring your scan reports, medication list, previous treatment history, and a clear description of what happens when you stand or walk. Your assessment may include a neurological examination and imaging such as MRI or CT. The clinician will compare the scan with your symptoms rather than treating an image alone.

Discuss physiotherapy, activity changes, medicines, or injections when appropriate, as well as the expected course without surgery. If an operation is proposed, ask whether it is decompression alone or decompression with fusion, how the robot changes the plan, what recovery restrictions apply, and how persistent or new symptoms would be handled. Tell the team about blood thinners, allergies, diabetes, smoking, and other conditions that may affect planning or healing.

Seek urgent medical assessment for new loss of bladder or bowel control, numbness around the inner thighs or genitals, rapidly worsening weakness, or severe difficulty standing or walking. These symptoms can signal serious nerve compression and should not wait for a routine surgical consultation. After surgery, urgent review is also needed for increasing weakness, a wound that is becoming red or draining, fever with feeling unwell, or severe pain that is not settling as directed.

Ask: Which nerve or level is compressed? What symptom is the operation intended to improve? Why is robotic guidance being considered for me? Would the same procedure be reasonable without it? Is fusion necessary, and what are the main risks and alternatives? What restrictions, wound care, and follow-up should I expect?
