Ringworm and diabetes: what to know

Ringworm is a fungal infection of the outer skin, not a worm. Diabetes does not create ringworm, but high blood glucose can affect the skin's defenses and make some infections harder to settle. A rash on the foot, leg, or another area may also be confused with eczema, irritation, or a bacterial infection, so appearance alone cannot confirm the cause.

Check the skin around the affected patch for spreading redness, warmth, swelling, cracks, drainage, or tenderness. A rash can be itchy and scaly without being severe, yet diabetes makes it sensible to arrange an earlier review if it is not improving.

Keep the area clean and thoroughly dry, especially between toes and in skin folds. Do not share towels, clothing, shoes, or personal grooming items, and wash fabrics that touch the rash. Avoid steroid cream on its own unless a clinician has advised it; steroids can change the appearance of a fungal rash and allow it to spread.

A clinician may recommend topical antifungal treatment for limited skin involvement or oral antifungal treatment when the area is extensive, recurrent, or difficult to treat. Tell them about diabetes medicines, glucose readings, and any kidney or liver condition before taking an oral medicine.

Arrange medical advice promptly if the rash is enlarging, painful, cracked, draining, unusually warm, or not improving with the advised treatment. Seek urgent care for rapidly spreading redness, marked swelling, fever, confusion, or feeling very unwell. A foot wound or infected-looking skin deserves prompt attention in someone with diabetes.

Ask whether the rash needs a skin examination or a sample, which antifungal is suitable with your medicines, and how long to continue it after improvement. Ask how to protect nearby skin and whether your glucose management needs review while the infection heals.
