Rheumatoid arthritis thumb: what to know

The thumb helps with gripping, turning keys, opening containers, and pinching. In rheumatoid arthritis, inflammation can make these actions sore or awkward. Symptoms may be most noticeable after rest, on waking, or during a flare, and may occur with finger stiffness or swelling in other joints. Thumb pain has several possible causes, so the symptom alone does not confirm rheumatoid arthritis.

Notice which joint hurts: the joint at the thumb base, the middle thumb joint, or the joint near the tip. Record visible swelling, warmth, reduced motion, and whether one or both hands are affected.

Use larger handles, avoid prolonged forceful pinching, and take short pauses during repetitive hand tasks. Gentle range-of-motion exercises can maintain movement when they are comfortable. A hand therapist may suggest joint-protection techniques or a supportive splint. Arrange review if pain interferes with work, sleep, or everyday tasks, or if swelling keeps returning.

Do not change prescribed rheumatoid arthritis medicines on your own. Ask about pain relief and whether anti-inflammatory medication is appropriate with your medical history and other treatments.

Seek urgent care for a suddenly deformed thumb, inability to move it after an injury, a hot swollen thumb with fever, rapidly spreading redness, or new loss of feeling or colour. Prompt review is also sensible when grip suddenly worsens or a flare is not settling with your usual plan.
