Short answer

Rheumatic fever can develop after an untreated or incompletely treated throat infection caused by group A streptococcus.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Trigger
A group A streptococcal throat infection can precede rheumatic fever.
Main concern
Inflammation may involve heart valves and joints.

What this means

The illness is an inflammatory reaction that can affect the joints, skin, brain, and heart valves. It is not the same as a throat infection, and it does not spread from person to person as rheumatic fever itself. The earlier throat illness may have included a sore throat, fever, tender neck glands, or a fine rash, but some people do not recognise it at the time. A fever can also have many other causes, including acute bronchitis, bronchiolitis, cellulitis, chickenpox, common cold, COVID-19, or croup.

Heart involvement may be temporary or may leave valve damage that needs follow-up. The connection is why a confirmed streptococcal throat infection deserves medical assessment and why treatment plans should be completed as prescribed.

What to do next

Arrange a clinical review if you have ongoing fever, migratory joint pain or swelling, unusual tiredness, chest discomfort, breathlessness, or involuntary movements after a recent sore throat. A clinician may review the history, examine the heart and joints, and choose tests based on the findings. Do not start leftover antibiotics or stop a prescribed course early. If rheumatic fever is diagnosed, treatment may include medicines for inflammation, management of heart symptoms, and preventive antibiotics over a period chosen by the treating team.

When to seek urgent care

Seek urgent medical help for severe breathlessness, fainting, new chest pain, a very fast or irregular heartbeat, confusion, or weakness that makes walking difficult. A child with a persistent fever, painful swollen joints, or new jerky movements should be assessed promptly. These signs do not prove rheumatic fever, but they can indicate a need for timely examination.

Questions for your doctor

Ask which earlier infection might explain the symptoms, whether the heart needs an examination or scan, how long medicines are needed, and which symptoms should trigger an immediate call. Ask how future sore throats should be tested and treated, particularly if you or your child has had rheumatic fever before.

Questions people ask

Is rheumatic fever contagious?

Rheumatic fever itself is not passed between people, although the streptococcal throat infection that can precede it may spread.

Can rheumatic fever affect the heart?

Yes. Inflammation can involve the heart and valves, so assessment and follow-up are important when rheumatic fever is suspected.
