Retropharyngeal cellulitis: what to know

The retropharyngeal space sits behind the throat and contains loose tissue that can allow infection to travel. Symptoms may include severe sore throat, fever, neck pain or stiffness, painful swallowing, drooling, a muffled voice, or difficulty opening the mouth. A clinician may use examination, blood tests, and imaging to check for swelling or a collection of pus.

A nearby throat, sinus, dental, or upper-airway infection can precede it. The condition is different from ordinary surface cellulitis, and it can deteriorate faster than its early symptoms suggest.

Seek urgent medical assessment and avoid eating or drinking if swallowing is unsafe or a procedure may be needed. Do not try to inspect or drain the back of the throat yourself. Hospital care may include airway observation, antibiotics through a vein, imaging, and drainage if an abscess is found.

Call emergency services for noisy or difficult breathing, inability to swallow saliva, drooling, blue lips, rapidly worsening neck swelling, severe drowsiness, confusion, or faintness. Fever with worsening throat or neck pain also deserves urgent review because infection can spread and lead to sepsis.

Ask whether the airway is secure, where the infection began, what imaging shows, whether drainage is required, how long monitoring may continue, and which swallowing or breathing changes should bring you back immediately.
