How to remove under-eye hollows

A tear trough is the groove between the lower eyelid and upper cheek. Its appearance may reflect facial structure, reduced soft-tissue fullness, skin thinning, or a hollow made darker by lighting. Dark circles can add colour to the shadow, while facial volume loss may make the transition look deeper. A hollow does not automatically mean that the skin is unhealthy.

Start with an in-person assessment that considers the hollow, lower-eyelid bags, skin quality, pigmentation, and cheek support. Under-eye filler may add support in selected patients, but this delicate area needs an experienced medical injector. Eyelid surgery can address excess skin or fat when anatomy calls for a surgical approach. Fat transfer uses fat from another body area to restore fullness. Results, suitability, swelling, and risks differ between approaches.

Arrange medical review if the change is new, clearly one-sided, associated with eyelid swelling, pain, redness, vision changes, or a skin lesion. Seek prompt care for sudden swelling with breathing difficulty or swelling of the lips or tongue. For a gradual cosmetic concern, a dermatology or oculoplastic assessment can clarify whether the main issue is a hollow, pigment, skin laxity, or another cause.
