What regression structures can mean

A dermatoscope magnifies a lesion and helps reveal colours and surface patterns that are difficult to see with the naked eye. Regression describes an area where pigment appears to have faded or been replaced by changes resembling a small scar. A clinician may describe white scar-like zones, grey dots, or blue-grey granularity as regression structures.

These findings are clues, not a diagnosis. They can occur in melanoma, but similar appearances may also be seen in benign moles or previously inflamed skin. Their significance depends on the lesion's overall symmetry, colours, borders, evolution, and whether the pattern is organised. Ask the examining clinician what the regression pattern looks like and how it fits with the rest of the lesion. They may recommend monitoring with a baseline image when the appearance is not clearly concerning, or removal and laboratory examination when the pattern, history, or change warrants tissue assessment. Do not scratch, pick, or attempt to remove the spot yourself.

When to seek care

Arrange a timely skin assessment if a mole or pigmented mark is changing, develops several distinct colours, becomes asymmetrical, bleeds without an obvious injury, forms a persistent crust, or creates a new sore. A regression description on a report is not, by itself, an emergency. The appropriate timing depends on the lesion's appearance and evolution, so follow the review interval given by your dermatologist and return sooner if new changes appear.
