What refractory means

Ulcerative colitis affects the lining of the large bowel and can cause diarrhoea, urgency, rectal bleeding and abdominal pain. The word refractory describes disease that does not settle adequately with the treatment being tried, or that flares again when treatment is reduced. It does not describe a personal failure or prove that no treatment will help. The reason may be ongoing inflammation, an infection, an incorrect diagnosis, missed doses, medicine intolerance, or a treatment that is no longer a good match. Symptoms and inflammation do not always change together, so assessment may include blood or stool tests and sometimes a bowel examination.

A flare can affect hydration, nutrition, sleep and daily activities. Your clinician may need to distinguish active ulcerative colitis from complications or another bowel problem before changing medicines.

Practical next steps

Arrange a review with your gastroenterology team or a GP, taking a list of medicines, doses, missed doses, side effects and recent symptoms. Record stool frequency, blood, night-time symptoms, pain, fever, weight change and ability to drink. Do not stop steroids or other prescribed treatment suddenly unless a clinician tells you to. Ask whether infection testing, inflammation checks, medicine-level monitoring or a different treatment approach is appropriate. Drink regularly, choose foods you tolerate, and seek advice before taking anti-diarrhoeal medicines or anti-inflammatory painkillers, because some products can worsen bowel symptoms.

When to seek urgent care

Seek urgent medical help for heavy or persistent rectal bleeding, severe or worsening abdominal pain, a swollen abdomen, repeated vomiting, fainting, confusion, high fever, or inability to keep fluids down. Contact your treating team promptly if diarrhoea is rapidly worsening, you are passing blood repeatedly, or symptoms are waking you at night. These warning signs can indicate significant inflammation, dehydration or another complication and need timely assessment.
