What persistent hiccups can mean

Hiccups are repeated involuntary contractions of the breathing muscle followed by a sudden closure of the voice box. Refractory hiccups are difficult to stop or keep returning. Reflux of stomach contents, gastritis, swallowing air, alcohol, a large meal, medicines, or irritation around the diaphragm can contribute. Less commonly, a nerve, chest, brain, or metabolic problem may be involved.

Long episodes can interfere with eating, drinking, sleeping, speaking, and breathing. The symptom itself does not identify one cause. A clinician may ask about recent illness, medicines, abdominal symptoms, neurological symptoms, and triggers, then examine you and decide whether tests are appropriate.

What to do next

Arrange a medical review when hiccups persist, repeatedly disturb sleep or meals, or return without an obvious trigger. Note when episodes begin, what you ate or drank, medicines taken, and whether heartburn, nausea, pain, or swallowing difficulty occurs. Sip water slowly, eat smaller meals, avoid alcohol and fizzy drinks, and sit upright after eating if reflux seems relevant.

Avoid frightening or forceful tricks that could make you choke. Do not start acid-reducing medicine or stop a prescribed medicine without discussing it with a clinician.

Seek urgent help if hiccups occur with severe chest or abdominal pain, trouble breathing, inability to swallow fluids, repeated vomiting, new weakness, confusion, a severe headache, or fainting. Prompt assessment is also needed if you cannot keep fluids down or become very weak, because dehydration and an underlying illness may require treatment.
