Red scarlet fever: symptoms and next steps

Scarlet fever commonly presents with a fine, rough rash that feels different from normal skin. It may be more noticeable in skin folds, while the face can look flushed and the area around the mouth may remain lighter. Fever and sore throat often occur, and swollen glands in the neck may be uncomfortable. The rash can change as the illness settles. These features suggest a pattern but cannot replace an examination, because several conditions can cause redness and fever.

Contact a clinician for advice about a new rough rash with throat pain or fever. Keep a note of the first symptom, temperature readings, rash progression, and any exposure to someone with a throat infection. Encourage fluids and rest, and choose cool, easy-to-swallow foods when needed. Do not use another person's medicine. If antibiotic treatment is prescribed, follow the instructions and ask what to do if vomiting or an allergy-like reaction occurs.

Get urgent help if breathing is difficult, swallowing saliva becomes impossible, the person is confused or very hard to wake, or signs of dehydration develop. Immediate assessment is also needed for a purple or bruise-like rash that does not fade when pressed, severe neck swelling, or rapid deterioration. Seek review if fever persists or the rash becomes painful or blistered.
