Red rash on the penis: what to know

Irritation can follow soaps, lubricants, condoms, sweat, friction, or poor tolerance of a product. Redness with soreness or moisture around the glans may reflect inflammation; itch and surface scale can occur with eczema or ringworm. A new medicine can produce a drug rash, and sexual exposure can be relevant when there are sores, blisters, discharge, burning urination, or swollen groin glands. These features overlap, so appearance alone cannot distinguish a skin condition from an infection.

Rinse with lukewarm water, dry gently, and avoid fragranced products, antiseptics, vigorous rubbing, and sexual contact if it causes pain or until an infection has been excluded. Wear loose underwear and keep the area dry. Do not apply steroid, antifungal, or antibiotic creams unless a clinician recommends the specific product. Arrange a sexual-health or dermatology appointment and mention recent products, medicines, sexual contact, circumcision status, and urinary symptoms honestly.

Seek urgent help for inability to pass urine, severe swelling, a foreskin stuck behind the glans, rapidly worsening pain, extensive blistering or skin peeling, or fever with marked illness. Arrange prompt review for ulcers, blisters, pus, discharge, bleeding, painful urination, groin swelling, or redness that is spreading. Persistent or recurrent redness deserves examination even when symptoms are mild.

Ask whether the findings fit contact dermatitis, eczema, ringworm, inflammation, or a sexually transmitted infection; whether swabs or urine tests are needed; and when sexual activity is safe.
