Red rash without fever: what to know

A rash may follow a new soap, detergent, cosmetic, plant, medicine, insect bite or heat exposure. Dry, irritated skin can become red and itchy without making you feel unwell. Some infections also begin locally or occur without fever, so the absence of a high temperature does not identify the cause.

Note where the rash started, whether it is itchy, painful, warm, scaly, blistered or swollen, and whether it fades when pressed. [Cellulitis](/conditions/cellulitis/) can make an area warm, tender and increasingly red. Viral illnesses such as [Chickenpox](/conditions/chickenpox/) or [COVID-19](/conditions/covid-19/) can have skin symptoms alongside other features, but a rash alone cannot diagnose them.

Take a clear photograph in good light and keep a note of new products, medicines, foods, travel and contact with anyone who is unwell. Wash gently, avoid scratching, and pause a newly introduced skin product. Wear loose clothing and use a plain moisturiser if the skin is dry and unbroken.

Do not apply prescription creams belonging to another person. Arrange an assessment if the rash keeps spreading, lasts, becomes painful, develops blisters or affects a child, older adult or person with reduced immunity. Get emergency help for a rash with breathing difficulty, swelling of the lips or tongue, faintness, confusion, or rapidly developing widespread hives. Seek urgent assessment for purple or bruise-like spots that do not fade under pressure, severe pain, blistering, skin peeling, eye or mouth involvement, or a rapidly hot and swollen area. Fever is not required for these warning signs.

Ask: Does the pattern suggest irritation, allergy or infection? Could a new medicine be responsible? Should I stop or continue a product? What changes mean I need urgent review?
