Red rash in the groin: what should you know?

Sweat and rubbing can inflame the groin fold, especially after exercise or in tight clothing. Intertrigo may look red, moist, tender, or cracked. Ringworm can have a scaly or raised edge that expands beyond the original area. Contact dermatitis may follow soap, detergent, wipes, pads, lubricants, or fabric. Psoriasis and atopic dermatitis can affect the groin with variable scale and itch. A drug eruption may appear in more than one body area after a medicine. Redness alone cannot separate these possibilities.

Clean gently, rinse well, and dry by patting rather than rubbing. Change damp underwear promptly and choose loose, breathable clothing. Avoid fragranced products, shaving, scratching, and sexual contact if the skin is open or infection is possible. Do not apply steroid or combination creams without guidance, since they can alter the appearance of some infections. Take a photo in good light and note the timing of sweating, products, medicines, or sexual contact. A clinician can decide whether examination or testing is needed and whether this resembles Intertrigo, Ringworm, or another condition.

Seek urgent help for rapidly spreading redness, severe pain, marked swelling, skin blackening, blisters, faintness, or breathing difficulty. Arrange prompt care for fever, pus, open sores, pain with urination, genital swelling, or tender groin lumps. Make an appointment if the rash persists, recurs, extends onto the thighs or genitals, follows a new medicine, or does not respond to basic dryness measures.
