Red nappy rash: what parents should know

Short answer
Red nappy rash is commonly caused by moisture, friction, or contact irritation, although persistent redness can also reflect a yeast infection or another skin problem.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Wetness, stool, friction, and fragranced products
- Helpful habit
- Frequent changes with gentle drying
What the redness may mean
Irritant nappy rash often affects skin covered by the nappy and may look red, sore, or shiny. Wetness, stools, tight nappies, and fragranced products can weaken the skin barrier. A yeast-related rash may be a stronger red with spots or small bumps beyond the main rash, especially when it does not improve with basic barrier care. Eczema, allergy, and bacterial infection can look similar, so the appearance and the baby’s general condition both matter.
Comfort and skin care
Change wet or soiled nappies promptly and rinse gently with lukewarm water or use fragrance-free wipes. Pat dry instead of rubbing, allow brief nappy-free time, and apply a protective barrier recommended for babies. Avoid talcum powder, scented products, and vigorous cleaning. If a clinician suspects yeast, they may recommend Topical Antifungal Treatment. Emollient Therapy can support a dry or irritated skin barrier when advised for the child’s specific rash.
When to seek care
Contact a clinician if the rash is not improving, keeps returning, spreads beyond the nappy area, or develops blisters, pus, open sores, or marked swelling. Seek urgent care if the baby is unusually sleepy, feeding poorly, has a fever, or appears seriously unwell. Do not use leftover steroid, antibiotic, or antifungal medicine without advice about the product and the baby’s age.
Questions for your doctor
Does this look like irritation, yeast, eczema, or infection? Which cleanser, barrier product, or medicine should I use, and for how long? What changes would mean the baby needs review?

Questions people ask
Can nappy rash be caused by yeast?
Yes. A yeast-related rash may persist despite routine barrier care and can extend beyond the main red area.
Should I use powder for nappy rash?
Avoid talcum powder because it can irritate the skin and create inhalation risk; use gentle drying and a suitable barrier instead.
