The short answer

Really dark under eyes are often caused by a combination of pigmentation, visible blood vessels, thin skin, facial shadows, irritation, or inherited features. The appearance can become more noticeable after poor sleep, dehydration, sun exposure, nasal congestion, or frequent rubbing, but it can also remain even when you feel well. Dark circles are a symptom or appearance rather than a diagnosis, so their colour and pattern matter when choosing care.

What very dark under eyes may mean

Brown or grey-brown colour often reflects extra pigment, including pigment left after irritation or eczema. Blue, purple, or reddish tones may show through thin skin when small blood vessels are more visible. A hollow beneath the eye can cast a deep shadow, making the area look darker even when the skin itself has little extra pigment; this is sometimes called a tear-trough hollow. Puffiness can also create changing shadows.

Itching, scaling, burning, or a history of rubbing points toward irritation or an inflammatory skin problem such as atopic dermatitis. Darkening on both sides that has been present for years is often linked to genetics, skin structure, or sun exposure. A clinician can separate these patterns by examining the skin, eyelids, and surrounding area.

What you can do

Use a gentle, fragrance-free cleanser and moisturiser around the eyes, avoid scrubbing, and resist rubbing when the area itches. Protect the surrounding skin from daylight with sunglasses and a suitable facial sunscreen, keeping products away from the eye itself. Regular sleep, adequate fluids, and management of congestion may reduce a tired or shadowed appearance, although they cannot change inherited hollows or established pigment.

If the colour persists, a dermatologist can assess whether pigment, vessels, irritation, or a hollow is the main contributor. Treatment may involve addressing dermatitis first, selected topical products, or a procedure chosen for the underlying pattern. Under-eye filler is used for some tear-trough hollows, but it is not appropriate for every type of dark circle and requires a qualified medical assessment.

When to seek medical care

Arrange a medical review if the change is new, marked, or mainly affects one side, especially when it comes with eyelid swelling, redness, pain, tenderness, discharge, a persistent rash, or significant itching. Seek prompt care for vision changes, severe eye pain, injury, or rapidly increasing swelling. These features need assessment because they can involve the eye or surrounding skin rather than ordinary cosmetic dark circles.
