What the pain may mean

Shin splints, also called medial tibial stress syndrome, usually cause aching or tenderness along the inner border of the shin after repeated running, jumping, or another impact load. A recent increase in training, hard surfaces, worn footwear, or altered foot mechanics can raise stress through the lower leg. Flat feet may change how the foot rolls and can contribute to overload, but shin pain can have other causes, including a stress fracture. A small, sharply tender area, pain at rest or at night, or pain that keeps worsening should be examined.

How to settle it

Stop running and jumping while ordinary walking is painful. Choose low-impact movement only if it stays comfortable, use supportive footwear, and return to impact in small stages after symptoms have settled. Do not use pain as a test of fitness. A physiotherapist can review your training load, calf and hip strength, balance, and running pattern. Gait retraining may help change a movement pattern that repeatedly overloads the shin; custom orthotics may be considered when foot mechanics are part of the assessment.

When to seek care

Arrange prompt assessment for pain in one precise spot, swelling, pain while resting, night pain, limping, numbness, or symptoms that do not improve after stopping impact activity. Seek urgent care after a significant injury, if the lower leg becomes very tight and painful, or if the foot becomes cold, pale, weak, or difficult to move. A clinician may need to distinguish shin splints from bone stress or another lower-leg problem.
