Short answer
Really bad cradle cap in an infant should be managed gently, with medical advice for inflamed, oozing, spreading or unusually uncomfortable skin.

What cradle cap looks like
Cradle cap, also called infantile seborrheic dermatitis, often appears as greasy yellow, white or brown scale on the scalp. It may extend toward the eyebrows, behind the ears or into skin folds. The scale can look dramatic without making a baby ill, and it is not caused by poor hygiene.
A very red, swollen, weeping or crusted scalp may indicate irritation, infection or another rash rather than uncomplicated cradle cap. Scratching or picking can break the skin and increase irritation.
Gentle home care
Use a soft brush only after loosening scale with a small amount of a baby-suitable emollient. Wash with a mild fragrance-free cleanser, rinse well and never force flakes off. Keep nails short and avoid adult dandruff products, essential oils and medicated creams unless a clinician has advised them.
If the rash persists or spreads, a clinician can examine the scalp and decide whether Emollient Therapy is enough. When a yeast-related component is suspected, Topical Antifungal Treatment may be considered with age-appropriate instructions.
When to seek medical care
Contact a clinician promptly if the scalp is hot, very red, swollen, painful, bleeding, oozing or foul-smelling, or if the baby has fever, seems unwell, feeds poorly or is unusually unsettled. Seek advice if hair loss, broken skin or a rash elsewhere is developing.
Questions for your doctor
Ask whether the appearance fits cradle cap, which cleanser and emollient are suitable for your infant, how to loosen scale safely, and whether any prescribed treatment should be used on the scalp or nearby skin.

Questions people ask
Is cradle cap caused by poor hygiene?
No. Cradle cap is an infant skin condition and does not mean the baby is being washed inadequately.
Can I scrape the scales away?
No. Scraping can injure the scalp; soften scale first and use a gentle brush only if the skin is calm.
