What should you know about raw nappy rash?

Short answer
Raw nappy rash is sore, inflamed skin that needs gentle cleaning, thorough drying, and a protective barrier.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common triggers
- Moisture, friction, and irritants
- First steps
- Clean gently and dry thoroughly
- Avoid
- Fragrance and vigorous rubbing
What the soreness can mean
Nappy rash, also called diaper dermatitis, develops when moisture, urine, stool, heat, and friction weaken the skin barrier. The area may look red, shiny, tender, or raw, with small cracks or spots. A rash in the folds, with satellite spots, or one that persists despite basic care can have a yeast contribution; the appearance alone cannot confirm the cause.
Broken skin is more vulnerable to irritation. Avoid fragranced wipes, vigorous rubbing, and products that sting. A clinician can distinguish ordinary irritant rash from infection, eczema, or another condition.
Practical care
Change nappies promptly, rinse with lukewarm water or use a fragrance-free cleanser, and pat rather than rub. Allow the skin to air briefly when practical, use a thick protective barrier, and choose a correctly fitting nappy. Wash hands before and after care. A bland moisturising approach such as [Emollient Therapy](/treatments/medical-dermatology/emollient-therapy/) may support the skin barrier.
Do not use steroid, antiseptic, antibiotic, or antifungal creams without suitable advice, especially on a baby. If yeast is suspected, a clinician may recommend [Topical Antifungal Treatment](/treatments/medical-dermatology/topical-antifungal-therapy/) and explain where and how to apply it.
When to seek care
Seek medical advice if the rash is raw over a broad area, spreading, blistered, crusted, oozing, or accompanied by fever, marked pain, or reduced feeding. Get prompt help if the child seems unusually sleepy or unwell. Arrange a review when careful nappy care has not begun to settle the rash or it repeatedly returns.
Questions for your doctor
Ask whether the pattern suggests irritation or yeast, which barrier product is suitable, whether any medicine is needed, and how long to try it. Mention recent antibiotics, diarrhoea, new wipes, and any rash elsewhere on the body.

Questions people ask
Should a raw nappy rash be left uncovered?
Brief air exposure can help when practical, but regular changes and a protective barrier remain central to care.
When might an antifungal treatment be needed?
It may be considered when yeast is suspected, particularly if the rash involves folds or does not improve with barrier care.
