Rash without fever: what to know

Possible triggers include contact with a new soap, detergent, cosmetic, plant, metal, or fabric; dry skin; insect bites; heat; hives; and viral or fungal skin conditions. Eczema-like rashes tend to be dry and itchy, while hives are raised and can shift location. A rash around one break in the skin that becomes warm, swollen, and painful may signal cellulitis.

The absence of fever does not settle the diagnosis. Describe the colour, texture, borders, itch, pain, scale, blisters, and whether pressing changes its colour. Note new products, foods, medicines, travel, pets, and contact with anyone who has a contagious rash.

Stop newly introduced skin products and wash gently with lukewarm water. Use a plain moisturiser if the skin is dry, keep nails short, and choose loose breathable clothing. Avoid scratching, harsh antiseptics, and sharing towels. Photograph the rash before applying anything new; this helps a clinician compare changes over time. A pharmacist can advise whether a non-prescription treatment is suitable for your age and health.

Get emergency help for breathing difficulty, swelling of the lips or tongue, faintness, a non-blanching purple rash, widespread blistering, skin peeling, or sores in the mouth or eyes. Arrange same-day care if redness is rapidly expanding, hot, very painful, associated with pus, or close to an eye. Make an appointment if the rash persists, repeatedly returns, disrupts sleep, or follows a new medicine.
