What should you know about rash in a fat fold?

Folds beneath the abdomen, breasts, or other areas can stay warm and damp. Rubbing then weakens the skin barrier, causing redness, soreness, burning, scaling, or small cracks. This pattern is often called intertrigo. Yeast or another infection may develop in the irritated area, while ringworm can form a spreading, scaly edge. Contact dermatitis may follow a new soap, fabric, deodorant, or cream. Atopic dermatitis, psoriasis, heat rash, and a drug rash can look similar.

The appearance matters: note whether the rash is moist or dry, sharply bordered or diffuse, painful or itchy, and whether it has blisters, drainage, or satellite spots. A clinician can inspect the fold and select treatment based on the cause.

Gently wash the area with lukewarm water and a mild, fragrance-free cleanser, then pat it completely dry. Change out of sweaty clothing, choose breathable fabric, and reduce rubbing with loose-fitting garments. Do not share towels, and wash clothing that touches the rash. Avoid fragranced products and avoid putting steroid cream on a suspected fungal rash unless a clinician advises it, because it can alter the appearance and allow infection to spread. Do not scratch or peel flaky skin. If the rash keeps returning, record triggers such as heat, exercise, new products, or tight clothing.

Arrange a medical review if the rash is spreading, repeatedly returns, or is not settling with gentle drying and friction reduction. Seek prompt care for increasing pain, warmth, swelling, pus, open cracks, fever, or rapidly worsening redness. A rash involving a large area, affecting someone with diabetes or a weakened immune system, or making movement difficult deserves earlier assessment.
