Rash from rheumatic fever: what to know

The rash associated with rheumatic fever is usually faint, irregular, and ring-shaped, with areas of clearer skin in the middle. It may come and go and is often found on the trunk or upper limbs rather than the face. A rash alone cannot confirm rheumatic fever, and many rashes have unrelated causes.

Clinicians look for the wider pattern: a recent sore throat, fever, painful or swollen joints that may move from one joint to another, chest discomfort, a racing heartbeat, unusual jerky movements, or small painless lumps beneath the skin. The illness is an immune response that can occur after certain streptococcal infections, so the history of the preceding illness matters.

Arrange prompt medical review if this rash follows a recent sore throat or fever, especially when joint, heart, or movement symptoms are present. Bring the names and dates of any antibiotics taken and a photograph if the rash disappears before the appointment. Until assessed, rest, drink fluids, and avoid strenuous activity if there is chest discomfort, breathlessness, or marked fatigue.

Seek emergency care for severe breathing difficulty, fainting, blue or grey lips, chest pain, a very fast or irregular heartbeat, new weakness, or a seizure. Same-day assessment is appropriate for swollen painful joints, recurring fever after a throat infection, uncontrolled movements, or worsening tiredness. Do not wait for the rash to become more obvious: heart-related symptoms can occur with little visible skin change.
