Rash and strep: what to know

Strep throat can present with a fine, rough-feeling red rash that often begins on the trunk and may spread. Sore throat, fever, tender neck glands, headache, or abdominal discomfort can occur alongside it. Strep bacteria can also cause skin infections, which may look different and may involve redness, pain, crusting, or drainage. A rash after starting an antibiotic may instead be a medicine reaction, so the timing matters. Conditions such as [Atopic Dermatitis](/conditions/atopic-dermatitis/) and [Contact Dermatitis](/conditions/contact-dermatitis/) can also cause rash without strep.

Arrange a medical assessment if a rash comes with a significant sore throat, fever, or exposure to someone with confirmed strep. A clinician may examine the throat and use a swab test when appropriate. Rest, drink fluids, and use only symptom medicines that are suitable for you and taken as directed. Do not share cups, utensils, towels, or toothbrushes. If antibiotics are prescribed, follow the instructions and report a new rash promptly, particularly if it is spreading or accompanied by swelling or breathing symptoms.

Seek emergency help for trouble breathing, swelling of the lips or tongue, faintness, confusion, or rapidly worsening hives. Obtain urgent assessment for a purple rash, blistering, skin peeling, severe pain, dehydration, inability to swallow fluids, or marked drowsiness. Contact a clinician promptly for a new rash after a medicine, high fever, worsening throat pain, or skin that is hot, draining, and increasingly tender.
