Rash after fever breaks: what it may mean

A post-fever rash can be part of a viral illness resolving. It may be widespread, mildly itchy and flat, or it may have bumps or blisters. Fever with cough or congestion can occur in common cold, COVID-19, bronchiolitis, croup or acute bronchitis. Chickenpox tends to produce itchy fluid-filled spots that appear in crops. Brucellosis may cause prolonged fever and general symptoms, while cellulitis is more often a hot, tender, localised area. A clinician may need to inspect the skin to separate these patterns.

Let the person rest and drink enough to pass urine regularly. Keep the room comfortable, use gentle unscented products, and avoid rubbing or picking the rash. Take a clear photograph in good light and record when the fever ended, when the rash began, and any new medicine or product. Do not share towels, clothing or personal items if an infectious rash is possible. Follow local school or workplace advice until contagious illnesses have been considered.

Seek emergency care for breathing difficulty, swelling of the mouth or face, fainting, confusion, a stiff neck, severe weakness, or spots that do not fade when pressed. Get prompt advice if fever returns, the rash becomes painful, warm or rapidly larger, or the person is drinking poorly. A newborn or young infant with fever and rash needs urgent assessment. Otherwise, book a review if the rash is not settling or its cause is unclear.

Ask whether the timing fits a viral rash, whether the person should avoid close contact, how to soothe the skin, and what change would warrant another examination.
