RANO and glioblastoma: what to know

Glioblastoma can change the appearance of brain tissue on MRI during and after treatment. RANO is a structured framework for describing whether enhancing tumour appears to have responded, remained stable, or progressed. The assessment may consider measurable enhancing tissue, new areas, swelling, non-enhancing change, steroid requirements, and neurological status.

An apparent increase in enhancement can sometimes reflect treatment-related inflammation or injury rather than straightforward tumour growth. For that reason, clinicians compare scans over time and interpret them with symptoms, treatment timing, and examination findings. The report is one part of a treatment decision, not a prediction made from a scan alone.

Attend scheduled MRI and oncology visits, and tell the team about headaches, seizures, weakness, speech or vision changes, confusion, balance problems, and any change in steroid dose. Keep a current list of medicines and note when symptoms began. Glioblastoma care may involve brain tumour surgery, radiotherapy, chemotherapy, or a combination, depending on the tumour and the person’s health.

Ask how the current scan compares with earlier images, whether treatment effects are possible, and what the next review will assess. A clinical trial may also be discussed when it fits the diagnosis and treatment goals.

Get emergency help for a first seizure, sudden one-sided weakness, new difficulty speaking, collapse, severe confusion, or a sudden severe headache with vomiting. Contact the treating team promptly for worsening headaches, repeated vomiting, unusual sleepiness, a new walking problem, or a noticeable change in behaviour.

Ask what the scan shows compared with the previous scan, which findings support the RANO interpretation, and whether inflammation or other treatment effects could be involved. Ask when imaging will be repeated, what symptoms require urgent contact, and how the result affects surgery, radiotherapy, chemotherapy, or trial discussions.
