What should you know about random spotting mid cycle?

Spotting may look pink, red, or brown and may appear only when you wipe or as marks on underwear. The amount and timing matter, but appearance alone cannot identify the cause.

Hormone fluctuations around ovulation, starting or changing hormonal contraception, missed contraception, or irritation after sex can cause bleeding between periods. Pregnancy-related bleeding is another possibility if pregnancy could occur, so mention this when booking an assessment.

Other explanations include infections such as Chlamydia, changes in the cervix such as Cervical Dysplasia or Cervical Cancer, and growths in the uterus or womb lining such as Uterine Fibroids or Endometrial Polyps. These conditions can present with bleeding between periods; spotting does not establish that any one of them is present.

A clinician may ask about your cycle pattern, contraception, medicines, sexual health, pain, discharge, and pregnancy possibility. Depending on your history, assessment may include a pregnancy test, examination, infection testing, or imaging.

Record when the spotting began, how long it lasted, how much you noticed, and whether it followed sex, a missed contraceptive dose, or a new medicine. Note cramps, pelvic pain, unusual discharge, fever, dizziness, or bleeding after sex.

If pregnancy is possible, take a pregnancy test according to the product instructions and tell the clinician the result. Do not stop prescribed contraception or medicines without advice. Arrange a routine appointment when spotting is new, keeps returning, occurs after sex, or does not settle.

Until you are assessed, use a pad or liner if useful for monitoring and avoid putting anything into the vagina if this increases bleeding or discomfort. Seek sexual-health testing if there is a new partner, discharge, pelvic discomfort, or concern about an infection.

Seek urgent medical help for heavy or rapidly increasing bleeding, severe or one-sided pelvic pain, fainting, marked dizziness, shoulder-tip pain, fever with feeling very unwell, or bleeding when you may be pregnant. These symptoms need prompt assessment, especially when they appear together.

If you have completed menopause, any vaginal bleeding or spotting should be assessed even if it happens once. Contact a clinician promptly for repeated spotting, bleeding after sex, or symptoms such as unusual discharge, pelvic pressure, or persistent pain.

Ask: Could my contraception, medicines, ovulation, or an infection explain this pattern? Do I need a pregnancy test, cervical examination, or infection tests? Which changes should prompt urgent care, and when should I follow up if spotting happens again?
