What should you know about QL muscle strain?

Short answer
A QL muscle strain affects the quadratus lumborum beside the lower spine and may cause one-sided back pain, tightness, or pain with movement.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What QL muscle strain means
The quadratus lumborum connects the lower ribs, spine, and pelvis. It helps steady the trunk and bend the body to one side. A sudden lift, twist, awkward reach, or repeated loading can irritate or overstretch its fibres. Discomfort may sit deep in the lower back or flank and can increase when coughing, walking, rolling in bed, or rising from a chair. A QL strain is different from a cramp, although protective muscle spasm can occur alongside it.
What you can do
Reduce the movement that triggered the pain, but keep gentle activity within comfort so the back does not become stiff. Short walks, supported sitting, and slow position changes may be easier than bed rest. A wrapped cold pack can help during the early sore phase; later, warmth may relax guarded muscles. Do not force stretching, deep massage, or heavy lifting while pain is sharp. A physiotherapist can check hip, spine, and trunk movement and build a gradual strengthening plan.
When to seek care
Arrange medical assessment if pain follows a significant fall, prevents normal walking, keeps worsening, or does not begin settling with simple care. Seek urgent help for new leg weakness, numbness around the groin, loss of bladder or bowel control, fever with severe back pain, or unexplained severe abdominal or flank pain.
Questions for your doctor or physiotherapist
Ask which movements should be limited, whether the pain fits a muscle strain, and when to restart lifting or sport. You can also ask how to distinguish QL pain from lower-back pain, back muscle spasm, or muscle cramps.

Questions people ask
Can a QL strain cause pain when walking?
Yes. Trunk stabilisation during walking can irritate the strained muscle, especially when stride or posture changes.
Should you stretch a painful QL muscle?
Only use gentle, comfortable movement at first. Stop if stretching causes sharp pain or symptoms that spread.
