Punch excision treatment: suitability and aftercare

A consultation comes first because not every scar has the same depth, edge, colour or cause. Punch excision is most relevant to selected small, depressed scars that can be approached with a circular instrument. The clinician considers the location, skin tension, active acne or inflammation and your history of unusual scarring.

During treatment, local anaesthetic numbs the area. The scarred core is removed, then the small wound is closed or dressed according to the plan. The purpose is to improve a defined contour; it does not smooth all nearby texture, prevent new acne scars or remove every colour difference.

Bring a complete list of medicines and supplements, and mention allergies, previous procedures, cold sores, skin infections and darkening after injury. Follow the clinic's wound-care instructions, keep hands away from the site and avoid pressure, friction and unapproved products. Protect the area from sunlight while it heals, and attend the scheduled check or stitch removal.

Contact the clinic if bleeding persists, pain intensifies, redness spreads, swelling becomes hotter or larger, drainage becomes pus-like, the wound separates or fever develops. Urgent care is needed for swelling that affects breathing or vision. Do not try to drain, cut or peel the area yourself.

Ask which scar is being targeted, what improvement is realistic, what mark the closure may leave, how you should wash and cover it, and what follow-up options exist if the texture remains uneven.
