What it may mean

The hamstring group runs along the back of the thigh and helps bend the knee and move the hip. A forceful sprint, kick, slip, deep stretch, or sudden change of direction can overload fibres high near the sitting-bone area. Bruising, swelling, a tender spot, or pain while stretching the leg can occur. A strain can range from overstretched fibres to a more substantial tear, so the exact severity cannot be judged from pain alone.

What to do now

Stop the movement that triggered the pain and use comfortable, gentle walking only if it does not increase symptoms. During the early phase, support the leg, use a wrapped cold pack for short periods, and avoid forceful hamstring stretching, massage, sprinting, and heat if they worsen swelling. As movement settles, a clinician or physiotherapist can guide gradual exercises for hip and knee motion, then strength and return to sport. Do not test the muscle with a hard run simply because the pain briefly improves.

When to seek care

Arrange prompt assessment if you felt a pop, cannot bear weight normally, have marked bruising or swelling, notice a visible dip, or cannot bend the knee against light resistance. Seek urgent help for severe pain after a fall, a cold or numb foot, or rapidly increasing swelling. If symptoms are not steadily improving, an examination can distinguish a hamstring strain from pain referred from the lower back or another thigh problem.
