Prostate cancer and lymph nodes: what to know

Lymph nodes are small immune-system structures that filter fluid. Prostate cancer cells can travel through lymphatic channels to nodes in the pelvis and, in more advanced disease, nodes farther from the prostate. A scan may show enlarged or suspicious nodes, but imaging cannot always prove why a node looks different. Your team may combine scans, examination findings, prostate-specific antigen results, and biopsy information when deciding the stage.

Node involvement does not describe the same thing as cancer in the bones or other organs. The location and number of concerning areas help clinicians plan care, while your general health and previous treatment also matter.

Ask which nodes are involved, how certain the finding is, and whether another scan or tissue sample is needed. Treatment may include hormone-blocking medicines, radiotherapy aimed at the prostate or selected nodes, or other systemic treatment. Surgery such as prostate removal is appropriate only for some situations. A specialist can explain how each option fits your stage, symptoms, and priorities.

Keep a record of scan dates, medicines, urinary changes, and questions. Difficulty passing urine should be reported because prostate or nearby-node disease can affect urine flow; it does not by itself establish cancer spread.

Contact your oncology or urology team promptly for new inability to pass urine, worsening blood in the urine, severe pelvic or back pain, new leg weakness, or loss of bladder or bowel control. Seek emergency help for sudden paralysis, severe breathlessness, or confusion. These symptoms can have causes unrelated to cancer, but they need rapid assessment.
