Propranolol dizziness: what should you know?

You may feel lightheaded when standing, unsteady while walking, or faint without the room spinning. Propranolol is a beta blocker; it reduces the effects of adrenaline and can make the pulse respond less quickly during activity. Dizziness may also be worsened by missed meals, vomiting, fever, hot weather, or not drinking enough. Other causes include anaemia, dehydration, low blood sugar, or an inner-ear problem such as benign paroxysmal positional vertigo. The symptom alone cannot identify its cause.

Sit or lie down as soon as you feel faint, then rise slowly when steady. Avoid driving, ladders, and other activities requiring balance until the symptom settles. Drink fluids if you have not been told to restrict them, and take propranolol exactly as prescribed. Do not stop it suddenly, because abrupt withdrawal can cause problems with the heart or blood pressure. Record when dizziness occurs, your dose timing, pulse if you can measure it safely, and any new medicines or illness. Arrange a medication review if it keeps returning.

Seek urgent help for fainting, chest pain, severe breathlessness, a new irregular heartbeat, confusion, one-sided weakness, trouble speaking, or a severe headache. Contact a clinician promptly for repeated near-fainting, a very slow pulse, falls, or dizziness that does not improve after resting. A clinician may review your blood pressure, pulse, dose, other medicines, and conditions such as atrial fibrillation or heat exhaustion.

Could my dose, timing, or another medicine be contributing? Should I check my pulse or blood pressure at home? What should I do if I miss a dose or become unwell? Could another cause, such as iron deficiency anaemia or hypoglycaemia, explain the episodes?
