What should you know about PROM in obstetrics?

The amniotic sac surrounds the baby and contains fluid. When it opens before contractions begin, fluid may leak as a gush or a continuing trickle. If this happens before the pregnancy has reached term, it is often called PPROM. The timing of the leak, the baby’s position, signs of infection and the wellbeing of both patients guide care.

Contact your maternity unit promptly, use a sanitary pad to observe the fluid, and note its colour, smell and when the leaking began. Avoid putting anything in the vagina until assessed. A clinician may check your temperature, pulse, the baby’s heart rate and whether the membranes have opened. Care can include observation, medicines or planning birth; antenatal steroid injections and induction of labour may be discussed when appropriate.

Seek immediate maternity advice for fever or feeling unwell, foul-smelling or green fluid, bleeding, worsening abdominal pain, regular contractions, reduced fetal movement or anything that feels suddenly different. These signs can indicate infection, labour or a concern for the baby and should not wait for a routine appointment.

Ask how the pregnancy stage changes the plan, how the baby will be monitored, which symptoms should trigger a return to hospital, and whether antenatal corticosteroid injection or induction of labour is suitable.
