What should you know about primary glioma?

Glial cells support, protect and nourish nerve cells. A glioma develops from one of these supporting cell families rather than spreading to the brain from an organ elsewhere; that is why it is called primary. Astrocytoma and oligodendroglioma are examples of glioma types.

Doctors describe a glioma by its microscopic type and grade, which indicate how its cells look and behave. They also consider where it is located, whether it can be removed safely, and molecular findings from tumour tissue. These details can affect the expected course and the combination of surgery, radiotherapy, medicines, rehabilitation and monitoring.

Assessment commonly includes a neurological examination and magnetic resonance imaging, often with contrast. If imaging shows a suspicious area, a neurosurgical team may recommend a biopsy or an operation to obtain tissue and relieve pressure when appropriate. The pathology and molecular report should be explained before treatment decisions are finalised.

Keep a record of new symptoms, medicines and changes in movement, speech, vision, memory or seizures. Treatment may involve observation for a carefully selected slow-growing tumour, surgery, radiotherapy, drug treatment, or a planned combination. Rehabilitation can support speech, movement, thinking skills and daily activities.

Seek emergency help for a first seizure, a seizure that does not stop, sudden weakness on one side, sudden trouble speaking, severe confusion, loss of consciousness, or a rapidly worsening severe headache with repeated vomiting. These signs can have several causes, but they need prompt assessment.

Contact the treating team promptly for a new or increasing seizure, worsening weakness or balance, repeated vomiting, increasing sleepiness, a change in behaviour, or new difficulty seeing, speaking or understanding. Do not drive after a seizure or while symptoms affect alertness unless a clinician confirms that driving is safe.

Ask which glioma type and grade are present, what the molecular results mean, and where the tumour is located. Ask whether tissue sampling or surgery is advised, what the main aims and possible effects of each option are, and whether radiotherapy or rehabilitation should be part of the plan.
