What should you know about previable PPROM?

PPROM means the membranes around the pregnancy rupture before labour begins and before the usual point at which intensive newborn care can offer a realistic chance of survival. Fluid may leak as a gush or a continuing dampness, but leaking fluid can have other causes. Assessment may include questions, examination, a sterile speculum examination, and tests chosen by the maternity team. The situation can affect fluid around the baby and may raise concerns about infection, cord problems, bleeding, or early labour. The outlook depends on gestational age, infection signs, bleeding, fetal wellbeing, and the condition of the pregnant person.

Treat suspected leaking fluid in pregnancy as urgent: contact your maternity unit or go to the recommended hospital now. Use a clean pad to observe fluid colour and amount, and avoid putting anything in the vagina unless the team tells you otherwise. Do not rely on a home test to decide whether care is needed. The hospital team will check you and the pregnancy, discuss observation or treatment, and explain options in language that supports your values. Antibiotics, monitoring, or an Antenatal Steroid Injection may be discussed at a stage when they are clinically appropriate. If continuing the pregnancy is not advised or labour begins, Induction of Labour may be discussed.

Seek immediate maternity care for fever or chills, feeling faint or very unwell, abdominal or back pain, regular tightening, vaginal bleeding, foul-smelling or green fluid, or reduced fetal movement when movements are expected. Call emergency services for heavy bleeding, collapse, severe pain, or trouble breathing. Do not wait for a routine appointment, because infection or labour can develop without a dramatic gush of fluid.
