What symptoms can mean
A tightening that comes and goes may be harmless, but a pattern that becomes regular, stronger, or painful needs checking. Pressure in the pelvis, a new low backache, watery discharge, spotting, or a gush of fluid can also occur when the cervix or membranes are changing.
Symptoms do not establish the diagnosis. At hospital, clinicians may monitor contractions and the baby’s heart rate, check your temperature and blood pressure, and examine the cervix or fluid when appropriate. They may ask about movements and any previous pregnancy complications.
What to do now
Phone your maternity unit immediately if these symptoms are persistent, repeated, or unusual for you. Note the timing of tightenings and the colour or smell of any fluid. Use a pad for leakage, avoid tampons, and do not place anything in the vagina before assessment.
If early birth is a concern, the team may discuss an antenatal steroid injection, monitoring, and treatments aimed at giving more time when that is safe. A cervical cerclage is considered only in particular cervical situations and is not an emergency treatment for every symptom.
When to seek care
Do not wait for a routine appointment if you have bleeding, a gush or continuing trickle of fluid, regular contractions, fever, severe pain, or reduced baby movements. Call emergency services for a strong urge to push, heavy bleeding, or a visible cord.
