What happens in preterm labor

During labor, contractions gradually change the cervix so the baby can pass through the birth canal. With preterm labor, this process starts earlier than expected. Tightening may feel like menstrual cramps, lower backache, pelvic pressure, or repeated abdominal hardening. A gush or trickle of fluid can mean that the waters have broken, and vaginal bleeding or a change in discharge can also occur. These signs can have other explanations, so an examination is needed to tell whether the cervix is changing.

The earlier labor begins, the more support a newborn may need after birth because organs, including the lungs, may still be developing. Some episodes settle after assessment, while others continue. What to do if you notice possible signs

Contact your maternity unit or obstetric clinician promptly if you have repeated tightenings, pelvic pressure, unusual backache, bleeding, or possible fluid loss before 37 weeks. Note when symptoms began, how often tightenings occur, and the colour or smell of any fluid. Use a pad for fluid rather than inserting anything into the vagina, and follow the local instructions about coming in for assessment.

Assessment may include checking your temperature and vital signs, listening to the baby, monitoring contractions, and examining the cervix. Depending on the findings and how far along the pregnancy is, care can include observation, medicines to support the baby’s preparation for birth, treatment for infection when present, or planning delivery if remaining pregnant is unsafe.

When to seek urgent care

Seek urgent maternity assessment for regular or painful contractions, a gush or ongoing trickle of fluid, vaginal bleeding, severe pelvic pressure, or new persistent lower back pain before 37 weeks. Seek emergency help if you have heavy bleeding, severe constant abdominal pain, feel very unwell, or notice markedly reduced baby movements. Do not wait for symptoms to become severe before calling, because early assessment helps the team decide what support is appropriate.
