Premature Adrenarche and Precocious Puberty: What to Know

Adrenarche is the rise in adrenal androgens that can bring pubic or underarm hair, body odour, or mild acne. Premature adrenarche describes these changes appearing earlier than expected without other puberty signs. Precocious puberty means the puberty process has begun early; signs may include breast development, testicular enlargement, a growth spurt, pubic hair, or vaginal bleeding, depending on the child.

The two patterns can overlap, but they are not interchangeable. A clinician considers the child's age, growth pattern, examination findings, family history, and possible exposure to hormone products.

Arrange a paediatric review if changes are progressing, several puberty signs appear together, or growth has accelerated. Bring a timeline of hair, odour, acne, breast or genital changes, growth measurements, medicines, and supplements. The clinician may monitor growth and development, examine the child, and request hormone testing or imaging when indicated.

A female hormone blood panel may be one part of the evaluation. Results need to be interpreted with the child's age, development, and laboratory ranges rather than viewed in isolation.

Seek prompt medical advice for vaginal bleeding, rapidly advancing puberty changes, severe headaches, vision changes, seizures, or a sudden marked growth change. These features do not identify a cause on their own, but they warrant timely assessment. Seek emergency help for a seizure, severe neurological symptom, or a child who is acutely unwell.
