Preeclampsia without swelling: what to know

Swelling is not required for preeclampsia. The condition is assessed mainly through blood pressure and signs of strain found in urine or blood tests, together with symptoms and checks on the baby. A person may feel well and notice no change in their hands, face, or ankles.

Because visible swelling is not a reliable way to rule preeclampsia in or out, attend screening even when your body looks and feels usual. Your clinician may repeat measurements, review medicines and medical history, and arrange fetal growth or wellbeing checks.

Keep antenatal visits and ask how to measure blood pressure correctly if home monitoring is advised. Write down readings, headaches, visual symptoms, abdominal pain, and fetal movement changes. Use prescribed treatment as instructed; avoid relying on the absence of swelling as reassurance.

Seek urgent maternity advice for a severe or ongoing headache, flashing lights or blurred vision, pain beneath the ribs, sudden shortness of breath, chest pain, vaginal bleeding, or less fetal movement than usual. These warning signs need assessment whether or not swelling is present. Call emergency services if you are severely breathless, faint, confused, or rapidly deteriorating.

Ask what your blood-pressure target is, which tests are planned, whether home readings are suitable, how fetal wellbeing will be followed, and whom to contact outside clinic hours.
