Pre-eclampsia pain under the ribs: what to know

Pre-eclampsia can affect the liver and cause discomfort in the upper abdomen, often under the right ribs. The feeling may be pressure, tenderness, or pain that does not settle. Pregnancy-related stretching, reflux, trapped wind, or a strained muscle can feel similar, so rib pain does not by itself confirm the condition.

A maternity clinician can compare your symptoms with blood pressure, urine protein checks, blood tests, and fetal assessment. Tell them whether eating, movement, or breathing changes the pain and whether it is getting stronger.

Call your maternity unit, obstetrician, or urgent pregnancy service as soon as possible if you have new pain under the ribs. Give the gestational stage, pain location, and any home blood-pressure reading, but do not delay care to keep measuring it. Rest while arranging help and bring your medication list if you attend a facility.

Follow the monitoring plan your team gives you. It may include repeat blood pressure checks, urine testing, blood tests, and monitoring of fetal growth or wellbeing. Treatment and decisions about birth depend on your findings and how far along the pregnancy is.

Go to emergency care now for severe or persistent rib pain, severe headache, flashing lights or loss of vision, sudden breathlessness, chest pain, repeated vomiting, fainting, confusion, seizure, heavy bleeding, or reduced fetal movement. Do not wait for pain to become unbearable.

Ask what tests are needed today, whether your pain could reflect liver involvement, what blood-pressure readings require immediate action, and how to track fetal movement. Also ask who to call overnight and how possible pre-eclampsia could affect the timing or method of birth.
