A PPROM test usually combines your history, a careful examination and checks of fetal wellbeing to determine whether amniotic fluid is leaking.

How clinicians assess PPROM

A clinician will ask when the wetness began, how much fluid you noticed, what it looked and smelled like, and whether you have pain, contractions, bleeding, fever or changes in fetal movements. They may check your temperature, pulse, blood pressure and the baby's heart rate.

A sterile speculum examination may allow the clinician to look for fluid in the vagina. A swab or other bedside fluid test may be used when appropriate. Ultrasound can assess fluid around the baby and growth, but it cannot by itself prove that the membranes have ruptured. Vaginal examinations are generally limited because they can increase infection concerns after suspected membrane rupture.

Preparing for assessment

Contact your maternity service before arriving if possible, follow their instructions and use a clean pad rather than a tampon. Bring your pregnancy notes and a list of medicines. Tell the team about allergies, previous PPROM, a cerclage, recent intercourse or any vaginal treatment, because these details can affect interpretation of the examination.

Do not delay assessment

Seek urgent maternity care for a gush or ongoing trickle, fever, chills, abdominal pain, contractions, bleeding, foul-smelling fluid or reduced fetal movements. A negative or uncertain first assessment does not make new symptoms safe to ignore; contact the maternity team again if leakage continues or changes.

Is a PPROM test painful?

The assessment may feel uncomfortable, especially during a speculum examination, but tell the clinician if you have pain so they can help.

Can a test miss a small leak?

A very small or intermittent leak can be difficult to confirm, so ongoing leakage or new warning signs should be reported for reassessment.
