PPROM at 18 weeks: what to know

A watery leak may be constant, intermittent, or absent after the first episode. It can be confused with urine or discharge. At hospital, clinicians may ask about the timing and colour of the fluid, check vital signs, examine you, and perform an ultrasound. Early membrane rupture can affect the fluid around the pregnancy and can be associated with infection or contractions. Your team will explain what the findings mean for you rather than assuming the outcome from the leak alone.

Phone your maternity service or go to emergency care now. Place a clean pad in your underwear and avoid tampons, intercourse, and putting medicines into the vagina unless prescribed. Do not drive yourself if you feel faint or unwell. Keep a record of bleeding, pain, fever, and fluid changes. A specialist may discuss observation, medicines, repeat scans, and the choices available if the pregnancy cannot safely continue. Steroid treatment is generally discussed in relation to a later stage when it may benefit fetal lungs.

Urgent treatment is needed for fever, chills, increasing pelvic pain, contractions, vaginal bleeding, foul-smelling fluid, fainting, or a sudden decline in your general condition. These are red flags for infection, bleeding, or labour. If you have only a possible leak, still seek prompt assessment because symptoms can be subtle.

Ask how the membranes will be assessed, what monitoring is recommended, what activity and hygiene advice applies, and which findings would change the plan. Ask who to contact at night and what emotional or bereavement support is available if decisions become difficult.
