PPROM at 14 weeks: what to know

Fluid may leak as a gush or a continuing dampness, but discharge or urine can feel similar. A clinician can examine the fluid, check your temperature and pulse, and use ultrasound to assess the pregnancy. At this stage, the amount of amniotic fluid may change, and the outlook is individual. Infection, bleeding, contractions, and changes in the baby's wellbeing are concerns the team will monitor.

Contact your maternity unit or emergency service immediately and explain that you may be leaking fluid. Do not put anything into the vagina, have sex, or use tampons while waiting for advice. Use a clean pad to observe the colour and amount, and take your maternity records and medicines list. Your team may discuss observation, antibiotics, follow-up checks, and what care is appropriate at this gestation. Later in pregnancy, an antenatal steroid injection may be discussed when it could help fetal lung development.

Seek emergency care for fever or chills, worsening abdominal pain, regular tightening, vaginal bleeding, foul-smelling or green fluid, dizziness, or feeling suddenly unwell. These signs can indicate infection, bleeding, or labour and need immediate examination. Do not wait for a routine appointment if the leak continues or you are uncertain whether it is amniotic fluid.

Ask how the diagnosis will be confirmed, which warning signs should trigger a return, how often monitoring is planned, and what choices are available if infection, labour, or bleeding develops. Ask what support is available for the emotional impact and who to call overnight.
