Recognising the pattern

Postpartum depression can involve persistent sadness, emptiness, guilt, loss of interest, irritability, fatigue, difficulty concentrating or changes in sleep and appetite. Some parents feel anxious, detached from the baby or overwhelmed by ordinary tasks. These experiences are not a sign of poor parenting, and a parent may have depression even when the birth went as expected. Depression can coexist with insomnia, daytime sleepiness or difficulty sleeping, especially when rest is already disrupted by infant care. A clinician can consider physical health, medication, feeding plans, previous mental health concerns and the support available at home.

Steps that can help

Tell a trusted person what you are experiencing and arrange an appointment with a GP, midwife, obstetric clinician or mental health professional. Ask someone safe to help with meals, feeds, appointments or rest. Keep basic routines where possible, including food, fluids and brief movement. Treatment may include talking therapy, medication, practical support or a combination; discuss breastfeeding and side effects rather than stopping a medicine suddenly. Depression is an illness, not a personal failure.

When to seek care

Seek urgent help if you are thinking about suicide, harming yourself or harming the baby, or if you cannot keep yourself or the baby safe. Hearing or seeing things others do not, extreme confusion, severe agitation or unusual beliefs after birth can indicate a psychiatric emergency. Contact emergency services or go to the nearest emergency department, and stay with a trusted adult while help is arranged.
