What should you know about posterior shoulder reduction?

A posterior dislocation is less common than a forward dislocation and can follow a seizure, electrical injury, or a forceful impact while the arm is turned inward. The arm may be held close to the body, with restricted movement and significant pain. Clinicians check circulation, sensation, and movement in the hand and arm, then use imaging to confirm the joint position and look for an associated fracture. Pain relief or sedation may be used. Reduction should be performed by a trained team because forceful attempts outside a medical setting can damage bone, blood vessels, or nerves.

Keep the arm supported exactly as instructed and avoid lifting, pushing, or testing the shoulder repeatedly. Attend follow-up even if pain improves; the shoulder may need repeat imaging and a plan for protected movement. Rehabilitation usually progresses from comfortable range-of-motion work to strengthening as healing allows. Physiotherapy can address stiffness, shoulder-blade control, and the muscle coordination needed for daily activity. Do not drive or return to sport until the treating clinician says your movement and control are adequate.

Treat a suspected dislocation as urgent, especially when the shoulder looks changed, the arm cannot move normally, or pain began after a seizure or major impact. Seek immediate help for a hand that becomes numb, weak, cold, blue, or unusually pale, or for pain that rapidly worsens after reduction. Fever, increasing redness, drainage, or new loss of movement during recovery also needs prompt review. Do not try to relocate the shoulder yourself.

Ask what the imaging showed, whether a fracture or tendon injury is present, and how long the shoulder needs protection. Ask which movements are allowed, when physiotherapy should begin, and what symptoms could indicate nerve or circulation problems. Before returning to work or sport, ask how strength and stability will be checked.
