Polymyalgia and rheumatoid arthritis: how are they different?

Morning joint stiffness can occur in inflammatory conditions, but its presence does not identify one diagnosis. The location of symptoms, visible swelling, duration and effect on function all add useful information.

Write down which joints hurt or swell, whether symptoms are symmetrical, how long stiffness lasts after waking and whether movement changes the discomfort. Photograph visible swelling if it comes and goes before an appointment. A clinician may examine your joints and request blood tests or imaging when appropriate. Gentle movement can prevent prolonged inactivity, but swollen or very painful joints may need a modified plan. Ask for advice on protecting hand function, sleep and work tasks while the cause is being assessed.

Arrange prompt medical review for new persistent joint swelling, rapidly worsening pain, difficulty using a hand or foot, or symptoms affecting several joints. Seek urgent help for a hot, very swollen joint with fever or feeling acutely unwell, because infection needs rapid assessment. Sudden severe weakness or loss of function also warrants urgent care.

Ask which features point toward PMR, rheumatoid arthritis or another cause; whether blood tests or imaging are needed; and how long you should monitor symptoms. Discuss treatment goals, safe activity, and which changes in swelling, pain or stiffness should trigger an earlier review.
