What should you know about polyhydramnios and preterm labor?

Extra amniotic fluid can increase the stretch inside the uterus. Some people notice tightening, pelvic pressure, backache or increasing abdominal discomfort. These symptoms can also occur for other reasons, so contractions alone do not establish preterm labor. Clinicians assess whether tightenings are regular, whether the cervix is changing, and whether the membranes have ruptured.

The amount of fluid, gestational age, fetal position and any underlying cause all influence planning. A scan can assess fluid and growth, while an examination is needed if labor or leaking fluid is suspected.

Follow the monitoring plan from your maternity team and note when tightenings begin, how often they occur and whether they settle with rest. Keep hydrated according to your usual clinical advice, and avoid self-treatment or medication changes. A growth scan may be part of follow-up for fluid and fetal well-being.

If you are before your expected delivery window and contractions become regular, call the maternity unit rather than waiting for the next appointment. The team may check your cervix, membranes, baby and signs of infection.

Call your maternity unit promptly for regular painful contractions, pelvic pressure that is increasing, a gush or trickle of fluid, vaginal bleeding, fever, severe abdominal pain or reduced fetal movement. Sudden severe breathlessness also needs urgent assessment.

Ask how your fluid measurement affects follow-up, how to distinguish practice tightenings from labor, which hospital number to use, and whether you need a cervical or membrane assessment. Ask what delivery planning would change if contractions or fluid levels increase.
