What should you know about PMS nausea?

Short answer
PMS nausea is a feeling of sickness that can occur before a period, often alongside cramps, bloating, headache or changes in appetite.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Pattern
- Often begins before menstruation and may ease when the period starts
- Helpful first step
- Sip fluids and eat small portions as tolerated
- Track
- Cycle timing, vomiting, pain, foods and medicines
The short answer
The timing matters: symptoms that reliably appear in the days before menstruation and ease after bleeding starts may fit a premenstrual pattern. Nausea can also have unrelated causes, so a new, severe or persistent symptom should not automatically be attributed to PMS.
What PMS nausea can mean
Hormone shifts before a period can affect digestion and interact with other PMS symptoms. Pain from cramps, migraine, stress, tiredness and changes in eating may add to the unsettled feeling. Some people notice reduced appetite, a sensitive stomach or an urge to vomit; others have nausea without vomiting.
Keep a simple record for a few cycles: when nausea starts, how long it lasts, whether vomiting occurs, where you are in your cycle, and any medicines or foods involved. This pattern can help a clinician distinguish PMS from problems such as acid reflux, gastritis, food poisoning or gastroenteritis.
What you can do
Take small, frequent sips of water or an oral rehydration drink, and try bland foods in small portions if you can eat. Crackers, toast, rice or soup may be easier to tolerate than a large meal. Avoid skipping food for long periods, strong smells and anything that you have noticed triggers nausea.
Rest, use gentle movement if it feels comfortable, and try a warm pack for cramps. Check the label before taking pain relief or an anti-sickness medicine, especially if you may be pregnant, have another medical condition or take regular medicines. A pharmacist or clinician can help you choose an appropriate option.
When to seek care
Arrange medical advice if nausea is new, keeps returning outside the premenstrual period, interferes with eating or daily activities, or does not settle after your period. Seek prompt help if you cannot keep fluids down, pass very little urine, feel faint or confused, have severe abdominal pain, blood in vomit, a high fever, yellowing of the skin or eyes, or a possible pregnancy with significant vomiting.
Urgent assessment is also sensible when nausea comes with marked one-sided abdominal pain, chest pain, trouble breathing or a severe headache. These features need assessment for causes beyond PMS, including gallstones, heat exhaustion, hepatitis A or hypoglycaemia.
Questions for your doctor
Ask whether your symptom timing fits PMS, whether another cause should be checked, and which treatments are suitable with your health history. Bring your cycle and symptom notes, including vomiting, pain, bowel changes, medicines, contraception and any chance of pregnancy.

Questions people ask
Can PMS cause nausea without vomiting?
Yes. PMS-related nausea may feel like an unsettled stomach or reduced appetite without actual vomiting. Recording when it occurs can show whether it follows a premenstrual pattern.
How can you tell PMS nausea from a stomach infection?
A stomach infection may begin independently of the menstrual cycle and may include diarrhoea, fever, abdominal cramps or contact with someone who is unwell. Repeated vomiting or difficulty keeping fluids down needs medical advice.
When should PMS nausea be checked?
Arrange an assessment when it is severe, new, persistent, occurs outside the premenstrual period, affects hydration or eating, or comes with severe pain, blood, faintness or possible pregnancy.
