Plastic Surgery Scar Revision: What to Know

Revision is planned around the scar’s width, direction, height, colour, texture, and position. A surgeon may remove part of the old scar and close the area in a way that places the new line along a natural skin crease or boundary. Other approaches can release tight scar tissue or alter its shape. The aim is improvement, not erasing the mark; the revised scar still needs time to mature.

Timing depends on healing, scar behaviour, skin tension, and the reason for the original operation. A raised, itchy, painful, or restricting scar may need a different plan from a flat scar that is mainly visible.

Bring details of the original surgery, previous wound problems, medicines, allergies, and any history of thick or overgrown scars. Your clinician should examine the scar and explain the proposed incision, anaesthesia, dressings, activity limits, and likely healing course. Do not start silicone products or massage until the wound has closed and you have been told they are suitable. A silicone gel sheet may be part of scar care after healing.

Keep the incision clean and protected as directed. Avoid stretching, picking, and unapproved creams while the skin is fragile. Sun protection can help reduce darkening of a new scar.

Contact the treating team promptly if redness spreads, pain worsens rather than settles, the wound opens, drainage develops, or you develop fever. Seek urgent assessment for heavy bleeding, sudden swelling, breathing difficulty, or signs of a severe allergic reaction. A scar that becomes increasingly raised, tight, itchy, or painful also deserves review because additional treatment may be needed.

Ask which feature of the scar the procedure is intended to improve, where the revised line will sit, and what alternatives may fit your skin. Clarify whether surgical excision is involved, how dressings and activity will be managed, and what signs should trigger a call. Ask how recurrence of a raised scar would be handled.
