What should you know about placental abruption without bleeding?

The placenta attaches to the uterine wall and supports the baby. If it separates early, blood may stay trapped between the placenta and uterus rather than pass through the vagina. This concealed bleeding can cause sudden abdominal or back pain, a tense or tender uterus, frequent contractions, or reduced fetal movement. The pregnant person may also feel dizzy or weak if blood loss is substantial. Not seeing blood does not show that the placenta is secure or that the problem is minor.

Call your maternity unit or emergency services for new persistent pain, painful tightening, reduced fetal movement, or feeling faint, even when underwear and toilet paper are clean. Lie on your side while waiting for help if that is comfortable, avoid driving when unwell, and report any fall, blow to the abdomen, or change in symptoms. Clinicians may check your vital signs, blood count and clotting, the baby’s heart rate, and the placenta with imaging. A normal scan cannot always exclude an abruption.

Get emergency help now for severe pain, a hard abdomen, fainting, breathlessness, confusion, cold clammy skin, reduced or absent fetal movement, or symptoms that are worsening. These warning signs can reflect maternal blood loss or fetal compromise without external bleeding.

Ask how the baby will be monitored, which blood tests are planned, whether observation in hospital is needed, what changes should prompt immediate return, and how delivery would be managed if the condition worsens.
