What should you know about pirfenidone rash?

Pirfenidone is used for some lung conditions, and a new rash after starting it or changing the dose deserves attention. A drug rash may be flat, raised, blotchy, itchy, or widespread. Its timing, shape, distribution, and accompanying symptoms help a clinician distinguish a medicine reaction from contact dermatitis, heat rash, psoriasis, or another skin problem. Do not assume every rash is caused by pirfenidone, but do not dismiss a new eruption either.

Take clear photographs in good light and note when the rash began, where it started, whether it is spreading, and any recent products or medicines. Use gentle, fragrance-free cleansing and moisturiser, avoid scratching, and protect irritated skin from heat and direct sun. Contact the prescriber promptly for instructions about the next dose; do not stop an important medicine or restart it on your own. A pharmacist can also help check non-prescription products for ingredients that may irritate the skin.

Seek emergency help for trouble breathing, swelling of the lips or tongue, faintness, blistering, skin pain, peeling skin, sores in the mouth or eyes, or a rapidly spreading rash. These features can signal a serious reaction and need immediate assessment. A mild, localised rash still warrants prompt contact with the prescribing team, particularly if fever or feeling unwell develops.

Ask whether the rash could be a drug eruption, whether pirfenidone should be paused or adjusted, which skin products are suitable, and which warning signs should send you to urgent care. Bring your photographs, medication list, and the date of your last dose.
